Facility Based Hospice Care

For almost 40 years, the Center has provided hospice services to terminally ill individuals throughout our community be it in people’s homes; nursing homes; other assisted living facilities; or, in one of our two unique, state-of-the-art residential facilities. As a mission driven, non-profit organization, no patient is ever denied service due to an inability to pay; as such, the Center provides, on average, over $3.5 million in charitable care each year (as verified by an external audit).

Both Center residences offer medically fragile individuals a truly home-like setting custom tailored to a patient’s unique situation and needs.

Our generously staffed facilities and limited number of residents allow for a level of attention unequaled by other more costly alternatives. Our staff of registered nurses, home health aides, cooks, housekeepers, volunteers, maintenance and clerical personnel are all there to guarantee that our residents’ needs, comfort and concerns are our top priorities. Residents and families alike can rest assured knowing that nurses and home health aides are on-site and available 24 hours a day, seven days a week.

All personal care is provided in the resident’s private room, each with an en suite bathroom. All hospice services are woven seamlessly into a warm, caring and respectful setting. Our residents can enjoy their privacy without ever having to be alone.

Families are able to relax knowing that the ever evolving challenges of caring for a terminally ill loved one are being professionally managed, allowing them to spend time visiting and not worrying.
